Care Coordination

TACOLCY is committed to keeping families whole and our Care Coordination program helps us do just that. Based on its core principle that nurturing families is the best guarantee for healthy, educated and happy children and youth, Care Coordination is a key element of TACOLCY’s priority to assist families to be effective caregivers and prevent their dissolution or loss of their children to the child welfare system. Each year, TACOLCY spearheads preventive and corrective services and follow ups to over 1,000 families in need. This program helps them have access to resources that assist with overcoming challenges, kicking substance abuse habits, learning parenting skills, getting job training, and making the changes necessary to keep their children with them. For each family, Care Coordination forms a committee of support (the family’s pastor or minister, a job counselor, a social worker, etc) to identify needed interventions to keep children safe and improve the quality of care they receive at home. Care Coordination also enhances the parents’ ability to provide economically and supportively in their nutrition, health, education and after school safety.
